Prior to your office visit, please read the HIPPA Notice of Privacy Practices and office policies form. Also print the Communication permission form and  the Client Information Sheet and Client Fact Sheet. Please complete them prior to your visit and bring them with you. This will save time on paperwork and allow  more time for your session.
